SCOM Report for a Specific Alert Name

I wanted the ability to produce a report for a specific SCOM alert name. I also want the ability to select resolution states and a start and end date range. I found the query that I wanted to use here, and modified it slightly to allow for parameters to be specified. The .rdl for this report can be downloaded here.
